What could be the cause of my toothache?
Toothache or tooth pain is most often caused by the dental pulp, or the ligament surrounding a tooth, becoming inflamed. This causes pain in and around the teeth and jaw. Dental pulp is a delicate tissue in the centre of a tooth that contains sensitive nerves and blood vessels and it can become inflamed for several reasons.
Toothache can range from mild, occasional discomfort to a severe, constant throbbing and the pain can be made worse by eating or drinking. There are also a number of conditions that cause pain similar to toothache. The actual cause can be correctly identified by your dentist or GP.
Common causes of toothache include:
Tooth decay – Also known as dental decay or dental caries, this is the most common cause of toothache and is caused by a build-up of plaque. This plaque produces acid, which breaks down the outer layer of a tooth and can eventually enter and damage the pulp within.
Cracked tooth/broken fillings – Occasionally, a tooth could crack or fracture for reasons other than cavities, such as by biting down on something unexpectedly hard. These cracks can lead to damage to the dental pulp if not attended to.
Receding gums – The later stages of gum disease causes the gums to start to shrink away from the teeth. This process can eventually expose the softer, more sensitive parts of the tooth root. Excessive brushing can also cause the gums to recede, causing a pain like toothache.
Periodontal/Periapical abscesses – A dental abscess is the result of a bacterial infection which causes a collection of pus, either within the dental pulp (periapical) or in the between the tooth and the gum (periodontal).
Abscesses within the tooth can develop as a result of dental decay breaking down the protective layers of the tooth, allowing bacteria to enter the dental pulp, or if the nerve dies as the result of an accident or injury. Periodontal abscesses (around the tooth) often develop as a complication of gum disease, where bacteria can form in the pockets of gum that have come slightly detached from the tooth.
Abscesses can cause swelling of the gums, swelling of the face, a severe throbbing pain, teeth and gums that are tender to touch, a high temperature and generally feeling unwell.
Wisdom teeth - If there is not enough room in your mouth, wisdom teeth may emerge from your gums at an angle or get stuck when they grow, resulting in what is know as impacted wisdom teeth (or third molars). There is often wisdom tooth pain, swelling and even infection, cysts and abscesses related to this condition. Normally it is a gum infection that causes the wisdom tooth pain. The pain radiates around the jaw making it feel like it is the ligament or bone around the wisdom tooth that hurts. Initial treatment for wisdom teeth is to irrigate and wash out the area with a disinfectant, with removal of the tooth being the last resort if the pain reoccurs every few months.
Sinusitis – Sinusitis is a viral or bacterial infection that causes an inflammation of the lining of the sinuses. Symptoms include a blocked or runny nose, pain and tenderness in your face and a high temperature. Sinusitis can also cause bad breath and toothache. Studies have revealed that sinusitis is commonly linked to other dental problems, such as gum disease or generally poor oral health.
Jaw joint dysfunction (TMJ) – This condition can be caused by the grinding of teeth, rheumatoid arthritis or gout and can result in tightness or spasms in your jaw muscles. Symptoms can include headaches, toothache, as well as difficulty opening your mouth or eating food.
Teeth grinding or clenching (bruxism) – If you grind your teeth, it can bruise the ligament around a tooth and the sensation can resemble toothache. Around 1 in 4 people are estimated to have problems with teeth grinding, and it is often caused by stress. In this case, a nocturnal bite guard can help. Resolving the underlying stress condition should also be a priority.
How can I treat my toothache?
Not all cases of toothache will require dental treatment, and it is important to get a proper assessment of its cause before you book a particular treatment. The most common treatments linked to toothache include:
Wisdom tooth removal – If your toothache is caused by an impacted wisdom tooth, you can have it removed. After a local anaesthetic injection to numb the area, an incision will be made into the gum in order to access the tooth to extract it. Sometimes a small piece of bone that covers the tooth may also need to be removed. The incision will be stitched up and should heal within 10 days. Upper wisdom teeth are usually much easier to remove than lower wisdom teeth and recovery can be as quick as 4 to 5 days.
Tooth extraction – If a tooth is infected and there is no chance of saving it with a root canal treatment, the tooth may need to be removed. A local anaesthetic will be injected into the gum by the tooth and the dentist will widen the tooth socket and rock the tooth side to side until it is loose enough to pull out. The open socket will then either be stitched up or be allowed to clot and heal.
Most people do not feel anything during the treatment but recovery can require anti-inflammatory medication to help with discomfort and pain.
Root canal treatment – The space within your tooth that houses the dental pulp, blood vessels and nerves is called the root canal. If an infection has led to an abscess or the nerves have died, you may need root canal treatment. This procedure requires drilling a hole in the crown of the tooth, removing the infected pulp and cleaning the area, then filling the canal and sealing the hole.
Bonding – If a tooth has cracked allowing the root to be exposed, it is occasionally possible to repair the tooth with a composite bonding resin. This is a simple procedure, comparable to getting a filling or inlay/onlay.
How can I prevent toothache?
The most straightforward way to avoid toothache is to maintain good oral hygiene in order to keep your teeth and gums as healthy as possible. This means brushing your teeth twice a day with fluoride toothpaste, flossing between the teeth and moderating your intake of sugary foods and drinks. Smoking and excessive drinking can also lead to dental problems.
Ensure you visit your dentist as often as advised for a dental check-up so any signs of gum disease or tooth decay can the spotted and treated early and quickly.

I was very impressed with Govan Dental Care, and Dr Lambert in particular. I had a dental emergency and made the appointment at 7am on a Saturday morning for 12pm the same day. This must be one of the few dental practices in Glasgow to operate on a Saturday. I was greeted warmly and seen at the appointed time. Dr Lambert was very friendly and professional, and explained everything in detail, putting my mind at ease. My problem was resolved within 15 minutes. The price charged was extremely reasonable. I will definitely return for my regular dental checks and any other work that needs doing. Highly recommended.

The dentist discussed all my options with me, and provided a first class service, removing my painful tooth, which whilst their is some pain from the site, this was explained to me, advice given and assistance if required. (being a wisdom tooth). would definitely recommend if scared of the dentist or new to the area.
I have recently moved to Maxident and normally I have a horrific fear of dentists. Well not any more. My procedures were handling quickly, I was seen within 10 minutes of arriving and even got an appointment for the next working day as a new patient. I had a very friendly welcome at the reception. The surgery was clean and smart. The treatment room was airy and the friendliness of the staff put me at my ease. Dr. Bhogal was very helpful and gave some useful advice. In all the experience was very good.
I drove from Southampton to Oxford to attend my appointment as I am home for the summer. I was stuck in major traffic on the way but managed to arrive 5-10 minutes late (rather than a predicted 30 minutes as I took a diverted route to try and beat the traffic). I phoned the receptionist as soon as I got out of the car in Oxford to tell her that I was on my way but she rudely told me that because I was already 5 minutes late, by the time I arrived at the surgery I would be 10 minutes late and therefore would not be able to see the dentist. Normally, you are kept waiting in the waiting room so I'm not sure why on this occasion I was just turned away. I did explain that it was not my fault as I left with plenty of time from SOUTHAMPTON, but the receptionist was having none of it and made me rebook my appointment. Very disappointed with the customer service as I would have expected a little bit more flexibility.

I made an emergency appointment at the Moorgate clinic as I had chipped my front tooth; I am not a regular patient. I was able to get one the next day (I booked the previous evening) and at lunchtime. I arrived a little early and was taken early. Needed an emergency appointment after painful weekend... suspected impacted tooth. Very easy to navigate website on Sunday night and had a Mon. appt for 9:30 AM. Received excellent, down-to-earth advice from Dr. Fallon. Staff had organized a Polish-speaking dental assistant in advance of visit (as Polish is native language). Very thoughtful, helpful ... would highly recommend them. Prices reasonable.

In all the categories above, I would give them 5 stars. I would have liked more information about the condition of my teeth at the appointment but that is only a minor issue. Also it is quite difficult to gt through to reception.

Very friendly, pleasant, and competent dentist and dental nurse. There is no limit on time and/or care despite the cheap bill. I had everything done, including x-ray, without any fuss. Everything was explained gently and with care. The practice is clean and efficiently run. I will definitely keep it as my go-to dentistry.

On the plus side easy to register and a friendly service in a modern and clean practice. Communication from the practitioners was good and clear. In the minus column, I was overcharged on my bill which meant another visit to the practice to sort out the discrepancy which as a bit inconvenient and a filling got knocked out when having my hygiene session, not inconvenient at the moment but it might prove to be...

I am very pleased with the service from my dentist. I was in pain in an area of my mouth which had been hard to discern in the Xrays. Through his careful and experienced examination, the exact tooth was located and immediate treatment was given. On entering The Maxident Clinic I went to the reception where I was immediately greeted by the receptionists. They were very friendly and explained exactly what I had to do. As it was my first appointment I had to fill in the NHS form and personal details before seeing the dentist. The dentist and dental nurse came across as very professional and, after the taking of an X-ray he explained what needed to be done. As I had an abscess he prescribed Penicillin, and I have now been booked in to go back for the required extractions.

As a new patient, I found the online booking efficient and was very pleased to be able to get a next day appointment (booked mid afternoon the day before). The dentist was very proficient,showed sensitivity to my situation and was very gentle when performing the examination. She also explained the treatment I would require very clearly and put me at ease. I will now be much less nervous about my follow up visits.
Great dentist, explained the fillings I needed in great detail. Spend a lot of time with me. He only works on Tuesdays though, but I'm more than happy waiting til then to see him again. All other staff seemed just as professional. Overall pleased with the service.

Very good attention. However, the original reason for my appointment was to replace a filling but so far It has been replaced twice but it keeps falling off.

I highly recommend this dentist. I am not from the area, but I had to have a fractured tooth looked at right away. I read the other good reviews on the site, which made me choose this dentist. All the good reviews are true. In the office, I was made to feel very comfortable right away, all my questions were answered, the procedure was done carefully, and the dentist prepared a complete form with photos of my procedure for me to give to my travel insurance and my dentist at home. I was really pleased with the result.

There is a shared reception area and the receptionist from the other practise seemed less than friendly when approached for information. Other than that I have no criticism and, overall, had as good an experience as you might hope for at the dentist's.
What I liked about this place is that they have appointments on late evening Thursdays AND Saturday mornings, perfect for busy working bodies like most of us. The staff and dentist I seen were also very pleasant.
